A Waiver must be filled out by all participants of the date in order to participate in the date. Failure to fill out the waiver will prevent all parties from participating in the date.

I hereby acknowledge that full payment is required at the time of booking to Dream Date Designers and is non-refundable. If I need to cancel my date, I understand I will be allowed to reschedule to the closest available date, within one calendar year of my original scheduled date, but I will not receive a refund of the monies paid. Payments can be made via Square, PayPal or Venmo. I also acknowledge that Dream Date Designers may cancel my date due to unforseeable events. I will be notified of any cancellations and my date will be rescheduled to teh closest date available within one calendar year of my original date.

I hereby acknowledge this release from liability for accidental injury, death or illness which I may incur as a result of participating in any date. I hereby assume all risks connected therewith and consent to participate in this date.

I agree to reimburse Dream Date Designers for any equipment loss or damage caused by myself or my guests. I understand that I am responsible for theft of any equipment if left unattended. I understand that I am responsible for the good conduct of all guests and that there is NO SMOKING, VAPING or USE OF ILLEGAL NARCOTICS allowed inside any of Dream Date Designers date packages.

I hereby agree to indemnify and save harmless Dream Date Designers and its partners and assigns from any claim, action, liability, loss, damage, or suit arising from participating in Dream Date Designers date packages.This agreement shall unlimited as to amount or duration and shall be binding upon all signed participants.
